STUDENT REGISTRATION

Registering your child for the upcoming school year is very
important. You provide us with your child’s updated
information, receive schedules and pay fees.
This year we are giving you two options to register for the 2009-2010 school year. You may register on, Friday, August 7, 2009 from 9:00 am to 7:00 pm., at either location, Boyer Valley South or Boyer Valley North. If you would like to avoid the hassle of registering your child at school, you may try doing it from our web site. Follow the instructions below to print, complete and return the correct forms, along with the fees to Boyer Valley School, 1102 Iowa Ave,. Dunlap, IA 51529, or PO Box 406, Dow City, IA 51528, before August 7, 2009
Hopefully these two options will make registering your child for school more convenient for you and the school staff. Remember it is important to have this paper work done before school starts. Please do not wait until Open House or the first day of school.

- Free & Reduced: If you feel you may qualify for our Free & Reduced lunch program, complete the Iowa Eligibility Application form by following the Instructions for Completing Iowa Eligibility Application. Remember to complete everything in Part 1, 2, 3, and 4. Applications not completed will be delayed. If you do not want your name released to Hawk-I or Medicaid be sure to sign the back side. - Waiver: If you complete the Free & Reduced Lunch forms and qualify you may also qualify for a waiver of other school fees. Be sure to sign this form and return also.
- Insurance Waiver: The School District does not purchase accident insurance to cover injuries incurred by your child at school. Student Accident Insurance does offer supplemental policies for you to purchase. The insurance form shows you the coverage and cost, if you are interested pamphlets are available at either location, just give us a call. If you have adequate insurance coverage and are not interested in purchasing more, please put all students’ names on one form and sign.
- Student General Data: Please complete one for each student. Fill in all empty lines. Please note if you would like your lunch statement sent to your e-mail address.
- Field Trip /Picture/Video Parental Permission Form: Please complete one for each student PreK-8th grade.
- Enrollment and Program Card: Complete one for each new student.
- Athletic Pre-Participation Physical Examination: Complete one for each student 7th-12th going out for sports.
- Records Release: New students transferring from a different school must complete.

FEES: Below
are the fees for the 2008-2009 school year. Please make out
two checks and designate what they are
for. Make out one for your lunch account and one for the
total of the other fees, which would include text book fees, and if
applicable activity pass, academy fees, PreSchool fees and,
remember if you have a kindergarten student include the afternoon
kindergarten milk fee.
